Re-admission procedure

Requirements and procedures:

  1. The returnee should accomplish the student’s clearance (issued by the Office of Student Affairs).

  2. Return the accomplished clearance at the Office of Student Affairs and get the Re-Admission form.

  3. Note: For returnee(s) seeking re-admission to BSHRM, DHRM, BSCpE, BSEE and Education Program, the returnee is required to undergo interview and evaluation of grades, and comply to the following requirements:Pay the medical examination fee at the External and Business Affairs (EBA).

    1. Get the interview form (available at the office of student affairs);

    2. Proceed to the respective department of the course the applicant wants to take and undergo the interview and evaluation of grades on the respective registration adviser(s);

  4. Pay the medical examination fee at the External and Business Affairs (EBA).

  5. Proceed to the Campus Nurse to get the Referral Slip. All returnee(s) should undergo the following medical examinations:

    1. Chest X-Ray

    2. Urinalysis

    3. CBC

    4. Blood Typing

    5. Hepa-B Screening

    6. Physical and Dental Examination

  6. Present the referral slip to designated clinic/Lab.:Submit the original copy of medical examination results/or photocopy of the results at the campus clinic.

    1. PRIMACARE LAB (EPZA)

    2. SERENDIPITY (GABRIELA SQUARE EPZA)

    3. FAMOUS CARE (TANZA)

    4. STO. DOMINGO (TEJERO)

  7. Submit the original copy of medical examination results/or photocopy of the results at the campus clinic.

  8. Secure the signature of the Campus Nurse on the admission slip.

  9. Proceed to the Office of Students Affairs to get the Notice of Admission for enrollment.

  10. Present the Notice of Admission and the result of evaluation at the Registrar’s office to get his/her Registration form.

  11. The returnee will pay for his/her tuition fee and will receive his/her Official receipt. The applicant will also pay the mandatory amount of P100.00 for the Central Student Government and Student Publication. The Registrar will validate the registration form upon presenting the official receipt of the applicant’s paid tuition fee along with the receipt given by the CSG Officer.

  12. The returnee will present the validated Registration form to the External and Business Affairs Office.
